The way Curative care and prevention is currently, it has distinctively become a concept since. My practicum which will take place at the United Medical Center Hospital in Washington, DC- mainly on the acute setting, will be based entirely on a community health program by focusing more on Health-education, patient evaluation needs after hospitalization and previewing the environmental issues affecting the patient discharge.
My practicum site utilizes past and theoretical perspectives in healthcare prevention and nursing professionalism. These roles involve partnerships and alliance with communities and populations while addressing health, social conditions and glitches. However, population-oriented preventive care will be applied directly to families and individuals. Primarily, the data collected which would play a critical role in planning and implementing ideas to help improve the community, the state and the society at large.

My practicum site the United Medical Center South East DC faces the challenges of the implementation of the evidence-based public health and most especially, patient’s unwillingness on health education; patient’s assessment related to discharge appraisal of the needs of patients after hospitalization is critical. The review of environmental issues affects the patients before and after discharges. For instance, homelessness, lack of guardianship, low-income rate and inability to access affordable health care services and insurance coverage. UMC DC encourages citizens to get vaccinated against flu and illnesses. There is a need for a patient’s handout, internet-based articles that can help implement an effective EBPH in UMC appropriately
