Increasingly, health professionals will need to work with clients in a more interactive, collaborative style. 3. Examine health statistics and demographic data to identify rates of poverty, homelessness, abuse, etc. in the area surrounding your community or city.
a. What services are available through the federal government, the state, and the local community?
b. Survey the surrounding area to identify community resources, soup kitchens, health centers, food banks, and clothing distribution centers for these populations.
c. How do persons who need these services access them? Give specific examples.
d. Discuss eligibility requirements for available services and the methods used to evaluate the services.
4. After examining populations in your area, select a population you are interested in within your community or city.
a. Identify 5 agencies in the surrounding area that provide health-related services to this selected population.
b. For each agency identified in your community or city, discuss:
i. What specific services are provided?
ii. Who is eligible for services?
iii. How service is initiated
iv. The cost of the services
5. Review a copy of Health People 2020 and find Quality Assurance (QA) goals for care in the community. List these QA goals. Describe ways these goals could be met in your community or city.
6. Select a health situation that you have identified in your community or city. Using the multilevel framework for each of the 4 levels of client (individual, family, aggregate, and community) describe:
a. Illness care
b. Illness and disease prevention
c. Health promotion care
