One in three people are living with at least one chronic condition, such as hypertension, diabetes or depression. By 2018 nearly three million people, mainly older people, will have three or more conditions all at once. The NHS Mandate (2013) details how the NHS should provide care services for individuals with long term conditions. This will be achieved through preventing ill health, providing better early diagnosis and managing on going physical and mental health conditions, helping individuals recover from episodes of ill health. The aim of this module is to explore the key role played by nurses in both supporting individuals to self-manage their long term condition and the delivery, development and appraisal of services across GP surgeries, district nurses, care homes and hospitals.
At the end of the module, students will be able to:
1. Demonstrate a critical understanding of the nature and major causes of morbidity and mortality in adults living with one or more long term conditions, understanding how physical, emotional, intellectual and socio-political factors can have an impact on health, outcomes, and care needs.
2. Demonstrate an in-depth knowledge of the relevant current policy and national guidelines relevant to the delivery of long term care services.
3. Explore the phenomenon of long term conditions from the perspective of the client, their family and carers.
4. Evaluate the role and contribution of professionals, carers and clients to the management of long term conditions, recognising the changing paradigms within which they exist and the need for joined-up care.
5. Appraise strategies and interventions to support individuals living with a long term condition to self-manage, including the financial freedom to engage services.
6. Demonstrate a critical understanding of a range of assessment tools and outcome measures, and their application in a variety of care settings.
