Assignment #1: Community Assessment

NU450-2: Appraise population health and health risks using community assessment, epidemiologic data, and vital statistics

Introduction

A windshield survey is a kind of community assessment that provides the nurse knowledge of the community of interest. The characteristics of the community and relevant public health data provide valuable information related to the health and well-being of the population who live within it.  A community assessment is the equivalent of a head-to-toe assessment, but instead of assessing an individual you are looking at characteristics that define a community. Visit the websites listed below to guide you as you conduct a modified community assessment:

Find a friend or someone to drive you through the community. As you travel through your community, take note of features that highlight the community’s unique characteristics.

What to observe in a general community assessment survey:

Epidemiologic data and leading health indicators

Physical environment

Education

Safety and transportation

Politics and Government

Health and Social Services

Recreation

Communication

Economics

Differences among neighborhoods or areas of the community

Prepare a 15-slide (excluding title and reference slides) PowerPoint presentation that addresses the characteristics of the community that you assessed The final slide should highlight key indicators that reflect the health of the community.

The PowerPoint presentation should include photographs, illustrations, and bullet points. The speaker’s notes must accompany each slide and be at least 100 words in length. You must include at least three references, cited within the speaker’s notes and included on the reference slide

Assignment Requirements

Before finalizing your work, you should:

be sure to read the Assignment description carefully (as displayed above);

utilize spelling and grammar check to minimize errors.

Your writing Assignment should:

follow the conventions of Standard English (correct grammar, punctuation, etc.);

be well ordered , logical, and unified , as well as original and insightful ;

display superior content, organization, style, and mechanics ;

be double spaced and in 12-point Times New Roman font; and

use APA 6th Edition formatting and citation style.

Submit your Assignment to the Assignment #1 Dropbox before midnight on the last day of the unit.

Assignment #2: Practice Experience Log

This course contains Assignments where you will be required to submit a Practice Experience Log . This document will be used to record the number of practice experience hours and the type of practice experience activities. Students who fail to submit the Practice Experience Log to the Dropbox with the specified Assignment will post a failing grade (“F”) for the course as a whole, regardless of performance on Discussion questions, Seminars, and other Assignments.

Directions:

Meet with a community health leader or group in in your community concerned with population health and present a 1-page summary of your assessment.

Record the hours that you spent completing each aspect of this Assignment (preparation, reading, writing, presenting, etc.) on the Practice Experience Log . Use the same log as you complete this for each Assignment. By the end of this course you should have one Practice Experience Log complete with all of your entries.

Submit the 1-page summary and the Practice Experience Log to the Assignment #2 Dropbox.
