Class, Reducing healthcare costs is a topic or area of research that can be related to every specialty.
The area in which I work, sees many patients that have Medicaid or Medicare insurance, or are from low income parts of town. Even a few patients are from out of town and the insurance wont cover the costs. This becomes a burden to many patients and families. Healthcare in the Unites States costs far more than in any other nation (Fries et al., 2013). Healthcare costs approximately nine thousand dollars a person for a year (Fires et al., 2013). By reducing healthcare costs, the stress of financially affording a hospital stay can be reduced. Many of the residents at the hospital I work at, order labs every day. If a patient’s labs are normal, we should speak up and question to the resident if the labs are medically necessary. Unless there is a medical reason, or if the patient’s labs are out of normal range, unnecessary tests should be avoided.
This is prime example of wastage of medical resources and unnecessary blood draws for the patient. Also, being a patient advocate to send the patient home with prescriptions that are also medically necessary or find pharmacies that offer generic medications for a cheaper amount. I chose this topic because I personally had large medical bills that I was unaware that I was going to have. I did my research and made sure that physician’s office I went to was considered in-network. The physician coded my visit as a well-visit check- up. This was not covered by my insurance, so I was left covering an eight hundred dollar medical bill. Also, I had a patient that was admitted to the hospital for ten weeks. She showed me the hospital bills she was receiving before the insurance made adjustments. She was admitted for extended monitoring for high risk twins, it was necessary for her to have non-fetal stress tests three times a day. These non-stress tests that monitor the babies well being cost the patient over a hundred dollars a session.
These tests are performed on one monitor and are watched by the nurse. The monitor is basically paid for after a few sessions, yet each patient is charged over a hundred dollars to be connected to this monitor for a period of time. Most insurances will not pay this amount, so either the hospital absorbs the costs, or the patient has to pay out of pocket. This topic will improve my practice because I need to be more of a patient advocate and help take some of the finance stress of the patient. This is primarly done by making sure any tests, or procedures are necessary, and help patients cut costs like using their home medications rather than using the medications provided by the hospital. The health topic of the week I had chosen to discuss in my Health IT paper was an article called Intermountain CEO: Digital health doesn’t have to conflict with human touch. The article discussed how the patient wants to have more access to health tools but also wants an empathetic doctor (Sweeney, 2017). Intermountain Healthcare CEO has stated that it is possible for the patient to have both (Sweeney, 2017). He believes this to be true based that the health system is focused on keeping patients at home and out of the hospital (Sweeney, 2017). Being capable of treating patients at home is probably the most empathetic thing to do for that patient (Sweeney, 2017). More and more people are connected to the internet via cellphones and computers (Sweeney, 2017).
Informatics are now playing a larger role in the healthcare field and healthcare systems have to keep up with the demand of everything being available at the push of a button for a patient (Sweeney, 2017). CEO Harrison states that personalized patient goals instead of the traditional biomedical goals already set would be more beneficial in measuring progress of the patient (Sweeney, 2017). By adapting personalized goals, the patient seems to be more receptive and follows the treatment plan that is designed for the patient (Sweeney, 2017). I chose this article as my topic because informatics are always changing. New ways have to be developed to keep up with the demand from the patient. In order to talk to the doctor or medical group, a patient would have to call and make an appointment or request a phone call in return. That phone call can take days to return and sometimes the doctor is not able to return the call and a nurse will make the follow up call. Patients have more access to internet options and want more ways to look at their health records and be able to talk to their doctor. With the advancement of technology, most hospitals and doctor’s office are connected to a thing called my chart or my health record online. The patients are able to view doctor’s appointments, blood test results, shot records, and e-mail their doctor. With my chart, the patient also has the ability to link their children’s chart if they have children, to their charts. A patient is also able to order medications this way.
Being that I want to be an APN, I feel having the ability to connect with patients that are unable to come to the office will be beneficial. I think to help make this work, I would try to have 2 days a week that would be available to connect with patients via facetime or through an app on phones and computers. Of course if someone called during other office hours, I wouldn’t turn them away but they would have to understand a call would be returned at a later hour when I am available There are many things that would need to be figured out completely but I think being able to connect to a patient is the next step in providing care as informatics constantly evolves.
