In Module/Week 2, you will be required to identify and assess a personal health concern. You must provide 1–2 sentences for each of the template questions below. Cut and paste the following outline into your assessment paper (excluding parenthetical information). Please make sure that your answers are not written in bold.
PART 1
1. Identify a health-related issue that has been an area of concern in your life for at least 6 months. (If you cannot identify an issue that you would label as a “problem” then select a health-related component of your life which is not as healthy as it could be. For example, you might consider issues related to: Stress management, sleep patterns, nutrition, mental health, fitness, water intake, cholesterol management, disease prevention…etc.)
2. Describe the impact this issue/problem has on your overall quality of life.
3. Rate the seriousness of this particular issue/problem on a scale of 1–10. (With 1 being mild and 10 being urgent and dangerous.)
4. In light of this issue/problem, what specific health-related behavior change would you be most likely to make? (This MUST be specific and measurable—drink 40 oz. of water a day, walk 2 miles a day, sleep 8 hours a night, etc.)
PART 2
1. List any triggers that seem to inflame or fuel this problem.
2. List any behavioral risk factors which fuel this problem. (This includes risky or questionable actions, etc.)
3. List any environmental risk factors which fuel this problem. (This includes peers, influences, population trends, toxins, etc.)
4. Identify your greatest obstacle to change.
PART 3
(This section is based upon the information from the Heath & Heath text. So, your answers for each item below MUST include a corresponding citation from the text.)
1. What could you do to reduce “emotional hindrances?” (such as fear, personal desires, appetites, etc.)
2. What could you do to reduce “intellectual hindrances?” (ways in which your thoughts and plans may at times sabotage your plans for change)
3. How could you more effectively “shape the path” towards change?

1. Based upon what you have read and learned thus far in the course, what advice would you give to someone who is struggling with a similar health-related concern?
