Assessment Purpose

The purpose of this assessment is to give you practical experience in completing a case management plan, using the supplied template. It will also help you to identify needs, and write appropriate goals and strategies. The short answer questions are designed to expand you understanding of the processes involved in conducting case management with clients.

Assessment Details

Following an initial meeting with your client “Patricia” you have gained the information outlined in the case study below. Read the case study and then answer the questions below and complete a case plan utilising the template provided.
Patricia is a single female who is 45 years of age, living alone in a one-bedroom unit. She has been a librarian for 20 years but has recently been retrenched. She is concerned that she will no longer be able to afford the unit and may need to find emergency accommodation.
She presents as highly anxious and indicates that she has trouble making friends or forming relationships. She indicates on the first visit that she was severely abused during her adolescent years. Her mother was transient and became involved with numerous partners, some of whom physically and sexually abused her. Her family was very poor and two of her sisters were removed from the family and placed in foster care and consequently estranged from them.
Patricia indicates that she wants to find another job but because of her limited social skills, she is too nervous to complete applications and attend interviews. Patricia had a friend that she had connected with on Facebook. He was from an African country and she had planned to meet him when he visited Australia. Eventually, he indicated that his family had become ill and he asked Patricia for funds for medical expenses and to be able to visit her. She provided him with $3000 to pay for outstanding bills for his family and to pay for his trip to Australia. Once the money was transferred she did not hear from him again.
Since being retrenched, Patricia has found it more and more difficult to leave the unit. She finds she is suffering from insomnia and sleeping late. When she leaves the unit, she becomes tense and has developed physical symptoms of anxiety. Trips away from the unit have become less frequent and shorter. Approximately one year ago, Patricia went to see her local GP about her anxiety and he prescribed her anti-anxiety medication. Although the medication appeared to help at first she feels it no longer has the desired effect and consequently is dependent on the pills in order “to function normally”.
Patricia was very successful academically at school but is not particularly adept at outdoor activities and sports. She still corresponds with a very close school friend who now lives in Sydney. Two months ago she had lunch with her mother for the first time in a year and they were able to reflect on some of the problems that had been encountered during Patricia’s adolescence.
Patricia feels strongly that her experiences during adolescence are preventing her from forming positive and productive relationships. She says that she has on a number of occasions, picked the wrong man and that most relationships have ended in her being highly humiliated or feeling overly controlled.
Patricia would like to enter into adult education so that she could find a new vocation. However, she does not see how she can do this in the current financial circumstances.
Patricia has recently had a little more to do with her sisters, who have now married and have children. She enjoys being a doting aunt and often takes the children out to parks or the beach. She expresses the regret that she has not had children herself and now feels that this will not occur due to her age.

Questions:

1. Identify the presenting issues including both risk and protective factors.
2. List five priorities for behavioural change (in order of priority, giving a brief rationale).
3. Why is it important to obtain informed consent before making a referral and how do you do this?
4. What skills and knowledge are essential when negotiating with specialist services?
5. What steps would you take to identify the specialists services available in your local area and what are five key factors to consider as part of the referral process?
6. List three specialist services that would be appropriate for your client. Ensure you list the key information about each service that is needed before a referral can be made.
7. Identify an interagency network in your local area and list the agencies that attend and the frequency of meetings.
8. What key workplace documents would you need to consult and/or complete in order to follow referral guidelines and protocols?
9. What are four ways you could monitor and review the case plan and how would you incorporate it in your case management practice?

Case plan:

Develop a case plan for your client utilising the template below. Also, be sure to include the following points:
• Identify three immediate needs, three short-term needs and three long-term needs of this client with associated goals and strategies.
• Allocate each plan/strategy to an appropriate person (indicate who is responsible, eg. yourself, another agency whom you have referred or the client) and enter appropriate target and review dates.
