1. For this assignment, your boss has asked you to focus solely on forklift safety.
Using what you have learned about controlling hazardous energy, write a step-by-step procedure on how to prepare a forklift for maintenance (zero energy). Your procedure must be at least one page in length, while being descriptive enough that someone else could use it to perform the work safely on the equipment.

Your company has four forklifts used in the warehouse. Two forklifts have a 4,000 pound capacity at a 28 inches load center and two forklift have a 6,000 pound capacity at a 34 inches load center. Your warehouse regularly handle palletized loads with load centers of 34, 38 and 40 inches. You will need to show your calculations and answers for each of these forklifts.
Your total report back to your boss must be a total of two pages (one for the procedure and the other showing your calculations for the safe load capacities for the company’s forklifts), not including title and reference pages.

2. Confined Space Fatality Incident Review
Throughout the years, there have been a number of multiple-fatality incidents within confined spaces in the workplace. Choose one such event, and summarize what contributed to the incident. Utilize the knowledge that you have gained in this unit, as well as personal experience, to determine what could have been done to prevent the accident.
Your paper must be a minimum of one page in length. You must include a minimum of three sources. All sources must be referenced; paraphrased and quoted material must have accompanying APA
citations.

· a brief introduction to the article;
· discuss action strategies to combat workplace hazards within the article; and
· describe concepts of exposure assessment, hierarchy of controls, workplace monitoring, and medical surveillance could be used to prevent occupational injuries and illnesses.
