In Assignment One you will:
Illustrate the population. Who are the members of this population? Provide details, including contributing causes for the disease and/or past medical history, any familial or genetic components for the disease.
Evaluate the population, including current numbers, the seriousness of the disease, special needs to optimize health and independence.
Assess the need for formal case management. What are the current trends in case management (as discussed in your text) that support a case management model?
Argue the potential benefits to implementing a case management model, addressing economics, quality or life/care of the patient, impact on the social life of the patient and family.
Analyze why nurses should coordinate the case management plan of care? Based upon best practice, what specialized training and experiences would a nurse bring to this role?
Identify the other members that should be included on a multidisciplinary case management team and discuss what their role is and why they are valuable members.

In this assignment, you will be exploring the use of evidence-based practice and how it is incorporated into case management patient care guidelines, and pathways based on your chosen chronic illness from assignment one. You will identify and discuss current best practices, recommended published guidelines for treating or addressing the illness, and describe how clinical pathways help to address the importance of basing case management on current practice.
Provide support from a minimum of two scholarly sources. Paper should be 4-5 pages in length.

For assignment 3, you will enter Sentinel World® where you will conduct a Family Support Assessment. In addition, you will create a Discharge Plan based on your family assessment. Submit the completed Family Assessment and Discharge Plan documents for grading under assignment 3.
Enter Sentinel World® and click the map to locate the Health Department or use the direct link under Sentinel World® at the top of the course home page for Assessments. Complete the Family Support Assessment Form by asking the client predetermined questions. When you have completed the assessment, click on Save and Review to view a PDF document of your assessment. Click on the down arrow in the upper right-hand corner of the screen and save your document to your files.
You will create a fictitious patient based on your family assessment and use the attached Discharge Plan Template document to create a discharge plan for that patient. Save the Discharge Plan Template to your files. You can type right into the document, save it, and submit for grading. You are to complete all sections of the discharge plan: assessment, diagnosis/plan, education needs, financial worksheet, and the reflection and conclusion. You need to be as detailed as possible in filling out all the boxes. The reflection and conclusion section allows you to summarize the patient’s plan of care based on all proceeding information and to make recommendations for the plan of care for the patient. Reflect on what you learned in this assignment. What do you think about using this discharge plan, did it help you to consider areas you might not have previously included in a discharge plan?
